i.          Letter of approval from the Singapore Registrar             {Original & two (2) photocopies}

            of Citizens and ICA

ii.         Malaysian identity card                                                       {Original & two photocopies}

iii.        Singapore pink/blue identity card                                      {Original & two photocopies}

iv.        Malaysian Restricted and International passport             {Original & two photocopies}

            (including expired & cancelled passports)                     

v.         Malaysia/Singapore birth certificate                                  {Original & two photocopies}

NOTE:

Malaysian Original Birth Certificate must be laminated.

Those who have Singapore birth certificates are required to produce their Borang ‘W’ (Malaysian Citizenship Certificate) or ‘Pengesahan Taraf’  (Confirmation of Status) / ‘Perakuan Taraf’ (Certificate of Status).

vi.        Copies of Singapore citizenship certificate and Singapore passport  

vii.       Photocopies of parents identity card, birth certificate, citizenship certificate and passport.  These documents are required for those:

a.      without a Malaysian IC / Malaysian Citizenship Certificate

b.      one of the parents is not a Malaysian Citizen

c.      age 21 & 22

d.      born outside Malaysia

viii.     Those born before 31 August 1957 are required to          {Original & two copies}

           produce his/her original   Malaysian Citizenship Certificate

           or their  parent’s original Malaysian Citizenship  Certificate             

ix.        Three recent colour passport size photographs

x.         All photocopies of documents must be in A4 size

xi.        Renunciation fee: SGD$6.00 (cash)
