I. APPLICATION FOR MALAYSIAN PASSPORT
REQUIREMENTS:
a) Application Form [ Imm. 42(Pin.1/97) ] – provided by the Consulate
b) 2 copies of recent passport size photograph :
§ Blue background
§ 3.5 cm X 5 cm
c) Malaysia Mykad and one copy
d) Hong Kong ID / permanent status ID and one copy
Additional Requirement For Applicant Below 18 Years Old:
a) Birth certificate and one copy; or
b) Borang W and one copy (if born overseas)
c) Either parent’s Malaysia ID and / or Hong Kong ID and one copy each
Additional Requirement For New Born Baby Of Whom The Mother Is Non Malaysian:
a) Parent’s Marriage Certificate and one copy

II. BIRTH REGISTRATION
Malaysians born in Hong Kong can be registered at this office on the following conditions:
- Father of the child is a Malaysian Citizen
- Registration is made within one (1) year from the date of birth
REQUIREMENTS:
a) Baby’s passport-sized photographs (2 copies)
§ blue background
§ 3.5 cm X 5 cm
b) Baby’s birth certificate (bring original and 2 photocopies)
c) Parent’s passport (bring original and 2 photocopies)
d) Parent’s Identity Card (bring original and 2 photocopies)
e) Parent’s Birth Certificate (bring original and 2 photocopies)
f) Parent’s Marriage Certificate issued by Malaysia Government
(bring original and 2 photocopies)
g) Applicant has to fill in Form ‘D’ provided by the Consulate
h) The fee is HK$ 62.00
Note: Father and baby are required to be present at the Consulate.
The birth of the child has to be registered within 1 year from the date of birth. For
the late registration, the application will be referred to the National Registration
Department in Malaysia. The above requirements and process are still applied.

III. GOOD CONDUCT APPLICATION
REQUIREMENTS FOR MALAYSIANS
Under new procedures, the Good Conduct Certificate can be issued within one day without refer to Malaysian Authority, but the applicants must get their declaration form endorsed by a Notary Public and the item ‘21’ in the application form must be supported by employer/sponsor before submission to the Consulate.
a) Malaysia passport (bring original and 2 photocopies)

c) Malaysian Birth Certificate (bring original and 2 photocopies)
d) 3 pcs passport-sized photographs
e) Fill in 3 copies of application form

REQUIREMENTS FOR NON-MALAYSIANS
The same procedures are still applicable for Foreigners.
a) Passport (bring original and 2 photocopies of all pages with stamp of entry & exit in Malaysia)
b) 3 pcs of passport-sized photographs
c) Fill in 3 copies of application form
d) The fee is HK$42.00

IV. REGISTRATION AS MALAYSIAN RESIDING ABROAD
Malaysians living abroad are encouraged to register themselves at the nearest Malaysian Mission. This is a requirement and is clearly stated in the Malaysian International Passport. Malaysians who fail to register would face the risk of having their nationality revoked under Article 25(2) of the Federal Constitution.
Registering yourself would enable the Malaysian mission to get in touch with you during emergencies or to inform on activities such as the Merdeka Celebration.
Registration can be done by calling upon the Malaysian Mission or alternately request for the registration form via telephone, letter or email and send in the duly filled form by post to be registered.
Address, telephone numbers and email address of Malaysian Missions are available under the topic Malaysia’s Mission Worldwide.
For short visits, it is sufficient to register by telephone or email. Information required is as follows:
§ Name
§ Passport Number
§ Current address in the country
§ Date of arrival
§ Expected date of departure

V. MARRIAGE REGISTRATION
Under (Marriage and Divorce Act 1976 of Malaysia)
Marriage can be registered at the Malaysian Mission if at least one of the couple is a Malaysian citizen.
Non-Muslim citizens who are married in Hong Kong MUST register the marriage with this office within the period of six (6) months from the date of marriage. This is a legal requirement and for the marriage to be legally recognized in Malaysia.
REQUIREMENTS:
a) Marriage Certificate (bring original and 2 photocopies)
b) Passport of husband and wife (bring original and 2 photocopies)
c) Identity card of husband and wife (bring original and 2 photocopies)
d) Passport-sized photo of husband and wife (2 each)

- 3.5 cm X 5 cm
e) For divorcee, please bring along the Divorce Decree (bring original and 2 photocopies)
f) The fee is HK$ 42.00
Note: Husband and wife must come together in person for signing of documents.
(Borang KC06 and KC07)
Those who were married outside Hong Kong and wish to register their marriage with the Consulate-General of Malaysia in the Hong Kong SAR, they should get their marriage certificate endorsed (authenticated) by the Consulate of the relevant country in Hong Kong SAR.
If over 6 months period, there will be a penalty charge of HK$ 200.00 for the first year (one day late is considered 1 year penalty). For each subsequent year, the penalty will be HK$ 100.00 a year or part thereof

VI. NOTARIAL SERVICES
1. Witness signature
Must come in person and sign at the Consulate

2. Endorsement of translated document (Malay language to English)
In person or authorized with an authorization letter
Bring the original document and copy
Fee: HK$21.00 per translation/copy
3. Translation of Malaysia Driving License
In person or authorized with an authorization letter
Bring the original license and copy
Fee: HK$21.00 per translation
4. Certified true copy
In person or authorized with authorization letter
Bring the original document
Fee: HK$21.00 per signature/copy
Note: Under normal circumstances documents submitted for the above services could be collected on the next day after submission.

VII. APPLICATION TO RENOUNCE MALAYSIA CITIZENSHIP
Under Constitution of Malaysia – Article 24 Termination of Citizenship, the Government of Malaysia does not recognize dual nationality.
REQUIREMENTS:
a) Malaysia passport (original and 2 photocopies)
b) Malaysia Identity Card (original and 2 photocopies)
c) Malaysian Birth Certificate (original and 2 photocopies)
d) Confirmation letter of Naturalization as a Chinese National
from Hong Kong/Macau Immigration (bring original and 2 photocopies)
e) Applicant has to fill in a statement form (2 set) – provided by the Consulate
f) Applicant has to sign 3 copies of “Borang ‘K’” – provided by the Consulate
g) 3 copies of passport-sized photos

h) The fee is HK$ 21.00
Note: Item 1 and 2 are to be surrendered and item 3 will only be returned back
once approval is received with endorsement.

VIII. SINGLE STATUS
REQUIREMENTS:
a) Malaysia passport (to bring original and 2 photocopies)
b) Malaysia Identity Card (to bring original and 2 photocopies)
c) The applicant has to fill in a declaration letter and sign it
d) Come personally to apply.
e) For divorcee, please bring along original Divorce Decree together with 2 photocopies.
f) The fee is HK$ 21.00
Note: Applicants could appoint someone in Malaysia to apply this certificate at National Registration Department on behalf of the applicant. The completed form can be endorsed in the Consulate before submission to Malaysia. |
