Documents to be submitted to the Embassy or Honorary Consulate in your country of residence, in person.
A. DOCUMENTS
- Duly filled form IM42 (please see note below for instructions)
- Original passport (if any)
- Photocopies of passport, pages 1-8 and last page of
passport (manual passport) and pages 1-3 (machine readable passport)
- Original Malaysian identity card (if lost, please
produce police report in English) Please note that we do not accept the
old blue Malaysian Identity Card
- 2 photocopies of the Malaysian Identity Card (front
and back of IC on the same side of A4 paper, please do not cut the
paper!)
- 3 passport photos, front close-up. ( 2
photos for the application of passport and 1 photo for the registration
form for Malaysian residing overseas) Please note that photos by
coin operated machines are not acceptable.
- One photocopy of bank draft/cheque
- Copy of the Residence Permit Card or “personbevis” (for Malaysians residing in Sweden)
- Original Birth Certificate & 2 Photocopies (only if IC is lost)
ADDITIONAL DOCUMENTS REQUIRED FOR APPLICANT UNDER 18 YEARS OLD.
- Original father´s/guardian´s Malaysian
Identity Card and 2 photocopies. If guardian has the sole custody, a
letter from the court in Malaysia regarding the same must be produced
and 2 photocopies of it.
- Father´s/Guardian´s signature and thumbprint (right thumb) on Form IM 42, column E.
B. FEE
'To be paid in pounds (£) by bank cheque made payable to the High
Commission of Malaysia in London, Belgrave Square, London SWIX 8QT,
ENGLAND.
· 32-page passport for 5 years £50.00 (£25.00 for child under 12 years old)
· 64-pages passport for 5 years £100.00 (£50.00 for child above 12 years old)
· SEK3.00 for one application form
PLEASE BRING THE CHEQUE WITH YOU TO THIS EMBASSY.
C. POSTAGE
SEK 85.00 (in stamps/cash) to forward your application to London with registered mail
D. REGISTRATION FORM FOR MALAYSIAN RESIDING ABROAD
It is compulsory for Malaysians to register with the Embassy. The form
for Malaysian residing overseas can be downloaded from our website.
Please complete the form and submit it together with your
application for new passport.
E. LETTER OF UNDERTAKING & POSTAGE
If you wish to have both the cancelled and new passports sent to you by
registered mail, please add the postage (in cash/stamps) for registered
mail with acknowledgement slip. The postage fee for year 2006 is
SEK77.00 (100g envelope) Please check with the Embassy or post
office for the up to date postage fees. The Embassy will not be liable
for any mail that is lost, misplaced or damaged during dispatch.
NOTE
1. Form IM42 has to be filled, signed and thumb
printed (right thumb) in the Embassy or Honorary Consulate.
Please observe the opening hours, which is from 9am to 12 noon, Monday
– Friday except public holidays. Please call the Honorary Consulate
concerned for appointment as they are opened only on certain days and
hours.
2. Pleased be informed that postage charges increase
every year and therefore you are advised to check with the Embassy or
Post Office for the updated fees involved.
