LOST OF MALAYSIAN PASSPORT

 

The Jabatan Imigresen Malaysia views the loss of a passport as a serious offense.  The passport is the property of the Malaysian Government and the holder should keep it safely and protect it from loss or damage at all times.

 

All cases of loss of passport are required to submit to the Immigration Authority in Malaysia.  The time taken for the receipt of a decision and approval to issue you a new passport from that authority is expected to take a considerable amount of time.

 

If you have lost your Malaysian Passport, please present at the High Commission and submit the followings:

(Applicant to fill in an application form at the High Commission in Wellington)

 

  1. Kenyataan Kehilangan Atau Kecacatan Pasport Malaysia (Lampiran B Imm 203A Pindaan) – 1 copy.
  2. Borang Soal Selidik Kehilangan Pasport Malaysia – 2 copies.
  3. Laporan Kehilangan Pasport Malaysia – 1 copy.
  4. Police Report / Statement with details loss of the passport (from the nearest Police Station) Police Report without DETAILS will be rejected: Original and Photocopy.
  5. Birth Certificate – Original and Photocopy.
  6. Identity Card – Original and Photocopy (both sides).
  7. Photographs (3.55mm x 5.00mm) – 4 copies duly certified.
  8. Thumb prints (on the form enclosed) – 2 copies.
  9. Photocopies of pages 2 and 3 of the passport (if any).
  10. A letter from your employer (or Educational Institution) confirming that you are employed (or are studying) there.  Original and Photocopy.
  11. A letter from New Zealand Immigration Service regarding your residence status.  Original and Photocopy.
  12. Borang A – Permohonan Pasport duly completed.
  13. Please make an appointment with Consular Office to be present to submit your application.  Tel.: 04-385 2439 / 04-801 5659.

 

Please abide with the above instruction and use the checklist when submitting your application.  Incomplete application or does not comply with the above requirements will not be processed and will be returned.
