A.   REGISTRATION OF NEWBORN CHILD

Please bring/submit the following:

1.

Complete and sign the Registration of Birth application form,

(JPN.WN.02 - Borang D)

 

2.

Complete the Registration of Malaysian Citizens in China form

 

3.

Original and two (2) copies of parents’ valid passport

 

4.

Original and two (2) copies of parent/s Malaysian Identification Card

(front and back or certified true copy* from Malaysia)

 

5.

Original and two (2) copies of parents’ Malaysian Marriage Certificate

(or certified true copy* from Malaysia)

 

   

6.

Original and two (2) copies of child's P.R.China Birth Certificate

 

7.

A fee of RMB 66.00 in cash * only made payable to Consulate General of Malaysia

 

8.

One (1) recent passport size photograph of each parent

 

9.

One (1) recent passport size photograph of the child

 

 

Certified true copies accepted ONLY from Malaysia if obtained from:

(a)  Any Immigration office

(b)  Any Jabatan Pendaftaran Negara office

(c)  Wisma Putra

 

FOR LATE REGISTRATION OF BIRTH (If Child is more than one (1) year old)

1.     Prepare and submit Items 1-9 above.

2.     Complete the Permohonan Pengesahan Taraf form.

3.     Submit four (4) recent passport size photographs of the child.

 

Note:

a).       As per Jabatan Pendaftaran Negara Malaysia ruling, the Child MUST be present during the registration of birth at the Consulate.  You may either come with your child during the submission of the application OR when collecting the birth certificate (Borang W)

b). Registration of newborn child must be done within one year from the date of birth.  Failure to do so would entail delay in processing and the application has to be referred to Jabatan Pendaftaran Negara, Malaysia.  Applications can be completed and signed by either father or Malaysian mother

c).  Father of the child MUST be a Malaysian in order to register

d). Processing of regular registration of birth applications is approximately 1 week.  For Late Registration, processing may take 3 to 6 months or longer, as approval needs to be obtained from Malaysia

 

 

 

B.   REGISTRATION OF MARRIAGE

Please submit the following:

1.

Complete and sign the Borang Permohonan Mendaftar Perkhawinan Luar Negeri (v) (JPN.KC 06).  Place your signatures under each photo

 

2.

Complete the Registration of Malaysian Citizens in China form

 

3.

Original and two (2) copies of applicant's Passport and identity card (front and back page)

   

4.

Original and two (2) copies of Malaysian Birth Certificate

 

5.

Original and two (2) copies of applicant's P.R. China Marriage Certificate

 

6.

Two (2) notarized copies of Notarial Certificate of Marriage which has to be in bilingual and attested by Foreign Affairs Office of the Province issue

 

7.

A fee of RMB 44.00 in cash * only made payable to Consulate General of Malaysia

 

8.

Two (2) recent passport size photographs of the applicant

   

 

Certified true copies accepted ONLY from Malaysia if obtained from:

(a) Any Immigration office

(b) Any Jabatan Pendaftaran Negara office

(c) Wisma Putra

 

Note:

a). Please submit your application and make an appointment to sign your Marriage Certificate in front of a Consular Officer at the Consulate.

b). Both husband and wife must be present for this appointment

c).  Registration of marriage has to be done within six (6) months of the marriage in China. The penalty for late registration is RMB220.00 for the first six months and RMB110.00 for every six months for each subsequent year.

 

1.       The Application forms for Registration of Birth (JPN.WN 02 - Borang D) and Marriage Registration is not available for download.  You may collect it personally or send us a self-addressed stamped envelope with a short covering note requesting for the application form and requirements list.

2.  Registration of either newborn child and marriage in Chongqing, Guangxi, Guizhou, Sichuan and Yunnan Provinces MUST be notarized by a Notary Public Office in that particular Province. For other Provinces, please refer to the Provincial Authority
