APPLICATION FOR NEW MALAYSIAN PASSPORT
 
Purpose: This document is issued to Malaysian citizen for the purpose of traveling abroad. Malaysian citizen who still has a valid visa status in Japan and whose passport is due to expire may make an application for a new Malaysian passport. Parents of Malaysian child born in Japan may also make an application for a Malaysian passport for the child.
 
Application by a person whose status of residence in Japan is Temporary Visitor will not
be accepted
 
Form: IM.42 – Pin. 1/97 (available at the Embassy of Malaysia)
Fee: RM200.00 for a 32-page passport (RM100.00 for a child under 12 years old and a person aged above 12 but below 21 years old whose status in Japan is Student). Fee is to be paid in Japanese Yen
 
Validity: Five years
How to Apply: Application and collection must be made in person at the Embassy. Application by mail will not be accepted.
Application hours: 9:00 am ~ 12:00 pm
Collection hours: 2:00 pm ~ 4:00 pm
(The Embassy is closed on Saturdays, Sundays, and public holidays)

DOCUMENTS REQUIRED
·Form IM.42 – Pin. 1/97
·Malaysian Passport and 2 photocopies of all pages except blank pages
·Malaysian Identity Card and 2 photocopies of both sides
·Malaysian Birth Certificate and 2 photocopies
·Malaysian Marriage Certificate and 2 photocopies (for married applicants)
·3 recent photographs measuring 5 cm x 3.5 cm with white background (photos must be taken at a photo studio, vending machine photos or instant photos will not be accepted)
·Student Status Certificate and 2 photocopies (for students)
·Employee Status Certificate and 2 photocopies (for workers)
·Certificate of Alien Registration and 2 photocopies (not required for non-residents in Japan)
.Application must be made by a parent if the applicant is below 18 years of age. In such a case, the following documents are required as well:
·Parent’s Malaysian Passport and 2 photocopies of pages with personal information, passport number, status of residence and signature of holder
·Parent’s Malaysian Identity Card and 2 photocopies of both sides
·Parents’ Malaysian Marriage Certificate and 2 photocopies
* All photocopies are required to be made in A4 size paper
