APPLICATION FOR NEW MALAYSIAN PASSPORT
(MRP: MACHINE READABLE PASSPORT)

(Application can be submitted in person or via courier services) Your Checklist 1 Application Form – Borang Permohonan Pasport / Dokumen Perjalanan (IM.42-Pin.1/97). Please write details correctly and clearly in BLOCK LETTERS. Note: To Request an application form: Kindly send self-addressed prepaid envelope to the High Commission (attn: Consular Section). Application form(s) will be sent upon receiving your prepaid envelope. Internet downloadable form cannot be used. 2 Certified Photocopy of ALL pages of current Malaysian Passport - (refer Note A) 3 Certified Photocopy of MyKad and Birth Certificate (2 copies each) - (refer Note A). Lost document(s) must be supported by POLICE REPORT. Replacement is not available at the High Commission. Please refer to http://www.jpn.gov.my. 4 For applicants under 18 years old: 2 Certified Photocopy of : · MyKad · Birth Certificate · Parents’ Malaysian Marriage Certificate · Parents’ MyKad (Note: Father to sign Section E13(d) - tandatangan bapa / penjaga yang sah) 5 3 Passport size photographs (3.5 x 5cm), recently taken. Background of the photo has to be BLUE in colour. (Polaroid and Photo Booth will be rejected). 6 Fee** payable only in BANK CHEQUE/MONEY ORDER to Personal Cheque will be rejected. Multiple fees can be accepted with one bank cheque. 7 Right thumbprint MUST BE WITNESSED at the addresses below (refer Note B) & sign section 13d. 8 Letter from Employer or Educational Institutution confirming you are employed or studying (including Permanent Resident). Kindly inform by writing if self-employed or unemployed. 9 To justify the reason in writing if Passport has expired when submitting the application 10 To produce letter from NZ Department of Internal Affairs on your citizenship status if passport has expired for more than 6 months. 11 Please fill in the acknowledgement letter. SEND IN 2 self-addressed prepaid envelopes for acknowledgement and notification.

Processing time is about three months. The High Commission has no authority with regards to processing time, applicant is advised to kindly wait for notification. O P T I O N :
Urgent Process of Application (To be sent to Malaysian Immigration Headquarters, Putrajaya by applicant):
i) Follow requirements (1) to (11). ii) Provide a letter of request with justification. iii) Send in an A4-sized self-addressed courier bag for the return of processed application.

To enable the Ministry of Foreign Affairs and Malaysian Missions abroad to provide an optimum service or to disseminate information to Malaysians abroad, it is important that we know how to get in touch with you. Therefore, all Malaysians living abroad are encouraged to register themselves at the nearest Malaysian Embassy / High Commission. This requirement to register is clearly stated in the Malaysian International passport. Malaysian fail to register would face the risk of having their nationality revoked under Article 25(2) of the Federal Constitution.
