APPLICATION FOR REGISTRATION OF BIRTH OVERSEAS WHOSE FATHER IS A MALAYSIAN CITIZEN

Please note that a child must be registered within one (1) year of his / her birth.

1. Please complete the Birth Registration Application Form (JPN.WN02/ Borang D), 2 copies in Bahasa Malaysia using BLOCK LETTERS ONLY and submit with the following documents either in person, mail to the following address or via our Honorary Consulate.

2. Original Birth Certificate of child in English, issued by the authorities in country of residence and two (2) photocopies.

3. Two (2) recent coloured passport sized photographs of child. The face should cover the entire photograph showing their features clearly. (Please note that photos by coin-operated machines are not acceptable).

4. Original passport of parents and two (2) photocopies each of pages 1-3 (machine readable passport) and pages 1-8 (manual passport)

5. Original Malaysian Identity card of parents and two (2) photocopies each of the front and back. Please note that we no longer accept the old blue Malaysian Identity card.

6. Original Malaysian Birth Certificate of parents and two (2) photocopies each.

7. Original Malaysian Marriage Certificate and two (2) photocopies. (Please register with the Malaysian Embassy, if your marriage is not registered under the Marriage and Divorce Act (1976) of Malaysia.

8. Original Proof of Permanent Resident (PR) card of parents or extract from the Population Registrar issued by the local authorities and two (2) photocopies each. Document(s) must be in English.

9. The fee is only RM30.00 to be paid in Swedish Crowns and in cash.

10. Please add the postage fee for registered mail with acknowledgement slip (AVI) to return your documents. Please call the Embassy/Consulate for the up to date fees. The Embassy of Malaysia will not be liable for any mail that is lost, misplaced or damaged during dispatch.

11. Please note that all incomplete applications will be returned to the sender.

NOTE:

1. The Birth Registration Application Form D (JPN.WN02) is available at the Embassy/ Honorary Consulate. You may collect it personally or send us a self-addressed prepaid 100g envelope if you want it to be mail-delivered to you.

2. The application forms are be signed by the father before us, Honorary Consulate or a registered Notary Public. The child’s left and right hand thumbprints are to be appended on both the forms.

3. When filling in the name of the child in the forms, please write it accordingly to the Malaysian way and NOT the way used in countries of residence i.e. the surname comes first and NOT last. The Embassy of Malaysia will not be liable for any mistakes made by the applicant in the application forms.

4. You need not send the originals if the documents have been certified by the Honorary Consulate of Malaysia in your country of residence.

5. Please be informed that the Consular Section is open from 9.00 am to 12.00 noon for walk-ins, Monday- Friday (except Public Holidays). Please call the Honorary Consulate in your country of residence for their opening hours.

APPLICATION FOR REGISTRATION OF MARRIAGE OVERSEAS

1)    Please complete the Marriage Registration Application form (JPN.KC 06) and submit with the following documents to:

2)    Original Marriage Certificate in English from your country of residence and two (2) photocopies.

3)    Original Permanent Resident (PR) cards of you or extract from the Population Registrar issued by the authorities in country of residence and two (2) photocopies.

5)    Original Malaysian passports of you and your spouse and two (2) photocopies each of pages 1-8 (manual passport) or pages 1-3 (machine readable passport).

6)    Original Malaysian Identity cards of you and your spouse and two (2) photocopies each of the front and back. Please note that we no longer accept the old blue Malaysian Identity card.

7)    Two (2) recent coloured passport sized photographs each. The face should cover the entire photograph showing the features clearly.

8)    The fee is RM20.00 to be paid in Swedish crowns in cash.

9)    A self-addressed prepaid envelope (postage depending on weight of envelope) for the return of your documents. The Embassy of Malaysia will not be liable for any mail that is lost, misplaced or damaged during dispatch.

Please note that all incomplete applications will only delay the process.

IMPORTANT:

Couple that have married outside Malaysia must register their marriage with the nearest Embassy, within six (6) months from the date of marriage. Late registration of marriages exceeding six (6) months will have to pay a penalty of RM$100.00 for the first year and thereafter RM50.00 per year. All payments are to be made in Swedish crowns in cash and receipts will be issued accordingly.

NOTE:

1.    The Marriage Registration Application Form (JPN.KC 06) is available at the Embassy/Honorary Consulate. You may collect it personally or send us a self-addressed prepaid envelope if you want it to be mail-delivered to you.

2.    Please send the forms and the original documents for verification first
prior to your visit to this Embassy for the registration. You need not send the originals ff the documents are certified by our Honorary Consulate.

3.    Couples are required to appear in person to append their signatures on both the application forms and marriage certificate and therefore please make sure you have an appointment made with the Officer concerned prior to your visit to this Embassy.

4.    Please be informed that the Consular Section is open Mon-Fri, 09.00am to 12.00 noon, except Public Holidays.

Add of married name or change of name into observation page in Malaysian Passport

Married name and change of name in country of residence can be done by the Immigration Department of Malaysia in London High Commission Office. If you wish to have your name added into your passport please submit the following documents upon submission of your application of new Malaysian Passport:-

1.   Original and 2 copies of Malaysian Marriage Certificate.
2.   Original and 2 copies of Identity Card issued by the authority in country of
       residence.
3.   Original and 2 copies of the extract from the local authority in english
       regarding change of name. In Sweden, ´personbevis` is the proof.
4.   Any other relevant documents

NOTE:

1.   Original of documents are not required to be submitted if copies of the documents have been certified true copies by the Malaysian Honorary Consulate in country of residence.

2.   It is advisable to add the change of name at the same time when you apply for a new passport.

3.   If applicant wishes to have the new name in the 1st page of passport, applicant must get the Malaysian Identity Card changed first at the local ´Jabatan Pendaftaran Negara` in Malaysia. And before applying for a new Identity Card, the applicant must make a Sworn Declaration in the presence of a Commissioner of Oath in Malaysia. Please be informed that the name of bearer in 1st page of passport will follow according to the name in the Identity Card.

4.   Please enclose postage fees for return of passport(s) with registered mail, with acknowledgement slip (AVI).
