APPLICATION FOR A NEW/RENEWAL OF MALAYSIAN PASSPORT

 

A. Required documents:

1. Completed and signed copy of the Registration of Malaysian Citizens in the USA form;

2. Current Malaysian Passport (if applicable);

3. The applicant’s Malaysian Identification Card* and one copy (front & back);

4. The applicant’s Malaysian Birth Certificate* and one copy (if the applicant is under 12 years old); and

5. If the applicant is under 18 years old, kindly include:

i) Malaysian Identity Card Original & copy of either Father's or Mother's

   (or certified true copy*);

ii) Original & copy of the child's Malaysian Registration of birth (Borang W)

 

B. Fees:-

The payment must be in cash or money order** made payable to “Consulate General of Malaysia

 

Normal Applicants (Age 13 – 59 years old)         = USD70.00

Senior Citizens 60 years old and above              = USD35.00

Children (Age 12 years old and below)               = USD35.00

Persons with disabilities                                         = Gratis

** Money order is accepted ONLY FROM:

  i. The United State Postal Service (USPS);

 ii. Western Union; or

iii. MoneyGram.

 

C. Note

*If the original document of the Malaysian Identification Card or the Malaysian Birth Certificate is unavailable, the applicants have to present the Certified True Copy of the said document. Certified True Copies are accepted ONLY from Malaysia if obtained from:

a)            Ministry of Home Affairs, Malaysia; or

b)            any Immigration Department (of Malaysia) in Malaysia; or

c)            any National Registration Department (NRD) (of Malaysia) in Malaysia.

 

All applicants must submit their applications in person. We do not accept applications by mail.

 

 

REPLACEMENT OF LOST MALAYSIAN PASSPORT

 

A. Required documents:-

1.   Completed Report of Lost Passport application form (Form IM. 203);

2.   Completed Borang Soal Selidik Kehilangan (v) Passport Malaysia (Form IM. 42C);

3.   Completed Borang Permohonan Passport (Form IM.42);

4.   Completed Registration of Malaysian Citizens in the U.S.A. form;

5.   Applicant’s Malaysian Identification Card and two copies (front and back);

6.  Applicant’s Malaysian Birth Certificate and two copies (if applicable);

7.  Applicant’s Malaysian Citizenship certificate/card and two copies (for applicant who was born before 1957);

8.  U.S. Permanent Resident Card or any other document verifying applicant’s status in the U.S. and two copies (if applicable);

9.  Original letter from employer or school to confirm applicant’s present employment or studies and two copies;

10. Police Report indicating the loss of your passport and one copy; and

11. Two recent passport size photographs (white background).

 

Note:

If the original document of the Malaysian Identification Card or the Malaysian Birth Certificate is unavailable, the applicants have to present the Certified True Copy of the said document. Certified True Copies are accepted ONLY from Malaysia if obtained from:

a)            Ministry of Home Affairs, Malaysia; or

b)            any Immigration Department (of Malaysia) in Malaysia; or

c)            any National Registration Department (NRD) (of Malaysia) in Malaysia.

The Soal Selidik Kehilangan form (IM.42c) is not available for download. You may collect it personally and you may be subjected to an interview at the Immigration Office in Consulate General of Malaysia, New York.

 

 

REPLACEMENT OF DAMAGED MALAYSIAN PASSPORT

 

A. Required documents:-

1.   Completed Report of Damaged Passport form.

2.   Completed Borang Permohonan Passport (Form IM.42).

3.   Completed Registration of Malaysian Citizens in the U.S.A. form.

4.   Applicant’s Malaysian Identification Card and two copies (front and back);

5.   Applicant’s Malaysian Birth Certificate and two copies (if applicable);

6.   Original & Two (2) copies of applicant’s Malaysian Citizenship certificate or card (for applicant who was born before 1957);

7.   U.S. Permanent Resident Card or any other document verifying applicant’s status in the U.S. and two copies (if applicable);

8.   Original letter from employer or school to confirm applicant’s present employment or studies and two copies; and

9.   Two (2) recent passport size photographs (white background).

10. Applicant’s may be subjected to an interview at the Consulate General of Malaysia, New York.

 

B. Note

If the original document of the Malaysian Identification Card or the Malaysian Birth Certificate is unavailable, the applicants have to present the Certified True Copy of the said document. Certified True Copies are accepted ONLY from Malaysia if obtained from:

a)            Ministry of Home Affairs, Malaysia; or

b)            any Immigration Department (of Malaysia) in Malaysia; or

c)            any National Registration Department (NRD) (of Malaysia) in Malaysia.

 

 

EMERGENCY CERTIFICATE

 

A. Required documents:-

1.   Completed and signed application form for Sijil Perakuan Cemas (Borang IM.42);

2.   Applicant’s Malaysian Passport and one copy (all pages except blank pages);

3.   Applicant’s Malaysian Identification Card and one copy (front and back);

4.  Two (2) recent passport size photographs (white background); and

5.   Original or notarized copy of a confirmed airline ticket to Malaysia.

 

B. Fees of USD17.00

(Payment by cash or money order** made payable to “Consulate General of Malaysia”)

** Money order is accepted ONLY FROM:

  i. The United State Postal Service (USPS);

 ii. Western Union; or

iii. MoneyGram.

Immigration Office in Consulate General of Malaysia, New York is the only producing office of Malaysia Electronic Passport (e-Passport) in the United States of America. This passport requires applicant’s thumbprint which will be linked directly to the main server in Kuala Lumpur, Malaysia. e-Passport will enable the usage of 'auto gate' system which is located at entry/exit points in Malaysia. To facilitate the system, the applicant must come in person.
