1) Registration of Birth

 

 Malaysians born abroad can be registered on the conditions:

 ·        Father of the child is a Malaysian citizen

·        Registration is made within one (1) year from the date of birth

·        Date of birth/age

·        Address in home country

·        Address of relative that can be contacted

 Application has to be made by the parents with the child. Documents to be submitted:

 ·        2 copies of Registration Form D, JPN. WND 2 (original)

·        3 recent coloured photographs of the child, size 3.5 x 5.0 cm

·        Birth certificate of the child in English translation (original and 2 photocopies)

·        Passport of parents (original and 2 photocopies)

·        Identification cards of parents (original and 2 photocopies)

·        Birth certificates of parents (original and 2 photocopies)

·        Marriage certificate of parents (original and 2 photocopies)

·        Acknowledgment letter from the hospital where the child was born

 The Embassy would then issue Form W, a letter that note the birth of the child in Hungary or in the other accredited countries .A copy will be sent to the National Registration Department of Malaysia for its record.

 Note: 

For registration made beyond the period of one (1) year of the birth, process still applies.  The Embassy will forward the application to the National Registration Department of Malaysia for approval.  Feedback of the application (approved or otherwise) will be notified to the applicant.

 

2) Registration of Marriage

 Documents to be submitted:

 ·        1 copy of Registration Form, JPN. KC 06 can be downloaded at www.jpn.gov.my

·        1 copy of the D2 Form with recent coloured photographs of applicants, size 3.5 x 5.0 cm

·        Local Marriage Certificate with translation in English (original and 2 photocopies)

·        Passports of applicants (original and 2 photocopies)

·        Identification cards of parents (original and 2 photocopies)

 Note:

·        The marriage must be registered within 6 months from the date of the marriage. Any registration made after 6 months of marriage is considered as late registration. The fee for late registration  is RM100.00 for the first consequence 6 months and RM 50.00 for every six months thereof.
