CONSULAR SERVICES PROVIDED AT THE HIGH COMMISSION OF MALAYSIA

 

FOR SRI LANKAN & MALDIVIAN

 

1.    COLLECTION OF THE CERTIFICATE OF GOOD CONDUCT

 

Certificate of Good Conduct/Police Clearance is issued upon request by Sri Lankan/ Maldivian who resided in Malaysia for not less than 12 months (on work permit / student permit etc.). It can be applied online at www.kln.gov.my/e-consular

 

Required documents (for collection)

 

i)   Printout of Status Update page (status should be “Ready for Collection”)

ii)  Request letter from the applicant

iii) Original + copy of passport (front page and Malaysia’s permit page)

 

Charges

RM 20.00 (To be paid in local currency as per the Monthly Exchange Rates)

 

FOR MALAYSIAN

 

1. ISSUANCE OF EMERGENCY CERTIFICATE (LOSS OF TRAVEL DOCUMENT/PASSPORT)

 

It is advisable that you keep a photocopy of your passport at all times to facilitate you. Malaysian whose passport has lost/damaged/stolen may make a report to the High Commission and can obtain the Emergency Certificate.  Required documents are: 

 

i)  Form IM 42C (forms can be printed from http://www.immi.gov.my or can be obtained from the High Commission) and police lost from

ii)  Police report from local police station with English translation

iii) Malaysian Identification Document (IC/Driving Licence etc)

iv. 3 Passport size photos (white background)

v.  Covering letter to the Visa Officer

vi. Passport copy

vii. Ticket confirmation

 

Note: An Emergency Certificate will be issued by the Mission which would enable the person to travel to Malaysia (one way trip).

RM 50.00 (to be paid in local currency as per the Monthly Exchange Rates)

 

2. UNOFFICIAL TRANSLATION OF MALAYSIAN DOCUMENT

 

Malaysian can translate Malaysian document such as Birth Certificate, Marriage Certificate & Driving License at the High Commission.

 

Required Documents

 

i)   Original and copy of document(s) to be translated

ii)  Request letter from applicant

 

Charges

 

RM 20.00 (to be paid in local currency as per the Monthly Exchange Rates)

 

3. RENUNCIATION OF CITIZENSHIP

 

Malaysian can apply for renunciation of citizenship status through the High Commission under Article 14 of the Federal Constitution. The application will be submitted to National Registration Department (Jabatan Pendaftaran Negara) for approval. 

 

Required Documents

 

i)   2 copies of Form K (can be obtained from the High Commission)

ii)  Original Birth Certificate

iii) Original Identity Card

iv) Malaysian Passport

v)  Original Malaysian Citizenship Certificate / Card (if applicable)

vi) Copy of Citizenship Card or Passport obtained from another country

vii) 2 passport sized photos
