REGISTRATION OF DEATH IN SINGAPORE

Requirement
1. Please be informed that the following documents must be submitted to the High Commission
:
    i. Singapore Death Certificate: 2 Original copies (must be legalized by the Singapore Academy of Law only)
    ii. The deceased's Malaysian Identity Card: Original and 2 Copies
   iii. The deceased's Singapore Permanent Resident Card, Work Permit, Employment Pass or SPass) - Original and 2 Copies
   iv. The deceased's Malaysian Passport: Original and 2 Copies
    v. The deceased's Malaysian Birth Certificate - Original and 2 Copies
   vi. Embalming Certificate: Original and 1 Copy
  vii. Sealing Certificate: Original and 1 Copy
 viii. Application and Collection for Cremation (issued by the Singapore National Enviroment Agency): Original & 1 Copy

2. For Muslims buried in Singapore, the family must produce the Application for Lease of Burial Plot issued by the Singapore National Enviroment Agency (NEA) - an original and one copy.

3. If a body is to be sent back to Malaysia;
    i. Permission to Export issued by the Singapore National Environment Agency: Original and 1 copy
   ii. Permission to Import/ Export or Tranship the Remains of the Deceased issued by the Jabatan Kesihatan Malaysia, Unit Kawalan Penyakit Berjangkit (Centre for Deceased Control, CDC): Original and 1 copy. 

  iii. Burial Permit/ Permit Menguburkan issued by the Police Station in Malaysia: Original and 1 copy.

4. Photocopy of the applicant's (must be the next of kin only) Identity Card: Original and 1 copy


5. Registration Fee : SGD 20.00 (cash only)

NOTE:
1. Please contact your appointed undertaker for items (vi), (vii) and (viii).
2. The two (2) original copies of the Singapore Death Certificate must be legalised by the Singapore Academy of Law, located at 1 Supreme Lane Level 6, Singapore ( Tel:6332 4388), before presenting them to the High Commission for the issuance of the Letter of Confirmation of Death Abroad.
3. After the High Commission issues the Letter of Confirmation of Death Abroad (Pegesahan Kematian di Luar Negara), the next-of-kin is advised to bring this document and the original death certificate to any National Registration Department in Malaysia to update the deceased's record.

4. Collection will be available after five (5) working days.

For those who have obtained the Singapore digital death certificate, please printed two (2) copies and legalised them first with the Singapore Academy of Law before presenting them to us.
