Passport Renewal
LIST OF DOCUMENTS REQUIRED
FOR THE APPLICATION OF A PASSPORT RENEWAL
AT THE EMBASSY OF MALAYSIA IN ASHGABAT, TURKMENISTAN
1) Application form (Borang Permohonan Pasport, IM42) to be filled and the applicant’s right thumbprint to be impressed on the form;
2) Original Malaysian Identity/ MyKID card of the applicant and a photocopy of the front and back in one page;
3) Original Malaysian Birth Certificate of the applicant and a photocopy;
4) Applicant’s Malaysian Passport and a photocopy of pages with bearer details on it and pages bearing Turkmenistan residency permit / visa;
5) Three (3) recent colored passport sized photographs with plain white background only (dimensions: 3.5cm X 5cm and must according to ICAO standard and specification);
6) A photocopy of the Marriage Certificate of the parents of the applicant (if the applicant is a child);
7) A photocopy of the parent(s) identity card of the applicant (if the applicant is a child);
8) Photocopies of the passports of the parents of the applicant and a copy of a stay permit (if the applicant is a child);
9) Photocopies of the Malaysian Identity cards of the parents of the applicant (if the applicant is a child);
10) A photocopy of the parents’ custody (if the applicant is a child); and
11) Consular Fee: USD30.00 in cash.
Note: Application for a passport renewal at the Embassy must be done not later than six (6) months before the passport expiration.
