REQUIREMENTS FOR BIRTH REGISTRATION

 

 

  1. Registration form (Borang D). Application form is available at the Embassy;
  2. Two (2) children photograph size 3.5cm X 5.0 cm with blue background;
  3. Two (2) photocopies of parent’s passport;
  4. Two (2) photocopies of parent’s identity card (IC);
  5. Two (2) photocopies of parent’s valid visa;
  6. Two (2) photocopies of Timor-Leste’s birth certificate and translation;
  7. Two (2) photocopies of Malaysian marriage certificate;
  8. Two (2) photocopies of parent’s birth certificate;
  9. Registration fee: USD 20.00.
