APPLICATION FOR MALAYSIA INTERNATIONAL PASSPORT

 

The Malaysian International Passport is a valid document issued by the Malaysian Government for the purpose of travelling abroad.

 

Application for Malaysian International Passport can be submitted at any Immigration Office in Malaysia or Malaysian Representative Office abroad.  In Taiwan application can be submitted at:

The applicant must appear in person during submitting the application.

 

Documents required together with application:

 

1.

Current Malaysia Passport - (original and photocopy)

 

2.

MyKad (IC) - (original and photocopy)

 

3.

Birth Certificate/Form W - (original and photocopy)

 

4.

Taiwan ARC/APRC Card – (original and photocopy)

 

5.

A complete Passport Application Form (IM.42 – Pin. 1/97)) – which can be downloaded from http://www.imi.gov.my or can be obtained at the counter.

 

6.

Two (2) copies of recent photograph passport size (35 mm x 50 mm with white background). Please refer above portal for detail photo specification.

 

7.

Parent’s identification  (Birth certificate/ MyKad for applicant below 18 years)

 

8.

Other supporting documents:

 

a)       If studying : certificate of studying (English version) / valid student card

b)       If working : certificate of occupation (English version)

c)        If long staying or holding resident visa: Malaysia Marriage Certificate or Taiwan’s English version certificate which is notarized by Taiwan’s Court or Ministry of Foreign Affair of Taiwan.

 

9.

Payment :     

 

a)        NTD2,000 for adult

b)        NTD1,000 for child (under 12 years), senior citizen (60 years and above), disable person, student (degree level) under 21 years. Must present University Offer Letter and fee certificate. 

 

 

The applicant, who is under 18 years old, should be accompanied by a Malaysian father/ mother.

 

All photocopies should be in A4 size.

 

Please be informed that the new passport will be produced within 3 - 4 months as all applications will be forwarded to Malaysian Immigration Headquarters, Putrajaya, Malaysia for processing. The applicants will be informed by phone if the passport is ready.

 

Upon collection of passport, please bring along the following documents:

 

i.              Payment receipt

ii.            MyKad

iii.           Existing passport

APPLICATION FOR MALAYSIAN EMERGENCY CERTIFICATE

 

The Emergency Certificate is a document issued to Malaysian to facilitate their return to Malaysia. This document is issued to Malaysian Citizens who have lost their passports or passport expired while abroad, and to those Malaysian Passport cannot be issued within the stipulated period.

 

Lost of passport

 

A Malaysian who has lost his passport is required to immediately lodge a police report to the nearest Police Station / Immigration Authority where the passport is lost or stolen and to Malaysian Representative Office abroad.

 

Requirements for the Malaysian Emergency Certificate are as follows:

 

1.

A complete application form (obtainable from the office)

 

2.

A complete questionnaire form on the loss of the passport (will be provide by the office)

 

3.

Two (2) copies of recent photograph passport size (white background)

 

4.

Birth Certificate / Form W (if any)

 

5.

Extract copy of the Police Report (for the lost case)

 

6.

Original MyKad

 

7.

Parent’s Identification Card (for applicant below 18 years)

 

8.

Copy of lost passport (if any)

 

9.

Any relevant document as a prove of Malaysia Nationality

 

10.

Fee NT$600

 

 

The Emergency Certificate application will be processed within 5 working days. The holders of this document are advised to refer to National Immigration Agency, Taiwan for clearance before able to leave this country.
