Important Information
- Both mail and in-person applications are permitted.
- Application has to be referred to the relevant authorities in Malaysia for necessary clearance. Once your application has been approved, it will be forwarded to Embassy of Malaysia in Washington D.C. to be submitted to the US Department of State.
- Processing time will be within 4 to 6 months from the date of submission.
Application Form:
- Certificate of Waiver online registration (e-Consular)
e-CONSULAR online registration must be completed and printed
Supporting Documents:
- Three (3) copies of completed on-line Letter of No Obligation/Waiver Application
- Three (3) copies of your Malaysian Identity Card (front and back)
- Three (3) copies of your Passport (all pages).
- An introduction letter from your employer (for foreign nationals).
- Third Party Barcode page issued by U.S. Department of State.
Note:
- Processing Fee: USD5.00 payable by Money Order or Cashier’s Check to the Consulate General of Malaysia.
- Return Postage: One (1) prepaid Priority Mail Express Flat Rate Envelope (USD33.40) from your local U.S. Postal Service for return postage.
- Required documents : Enclose a copy of each set of document for the Consulate General's filing purposes
