Appointment Scheduling
To schedule an appointment, please send an email to mwlosangeles@kln.gov.my or lax.info@kln.gov.my
Kindly ensure that your email includes the following information:
- Full Name
- Contact Number
- City and State of Residence
- Subject Line: Deceased Malaysian abroad (please indicate the State)
Documents required are as follows:
- Original Death Certificate
- Original Certificate of Disposition of Human Remains
- Original Cremation Certificate (if any)
- Name and Address of Mortuary
- Original Passport of Deceased
- A copy of passport for the person accompanying the body / ashes
- A copy of Flight itinerary/ticket.
- For return of your passport and documents, provide one (1) prepaid Priority Mail Express Flat Rate Envelope (USD30.45) available from your local U.S. Postal Service.
