Appointment Scheduling

To schedule an appointment, please send an email to mwlosangeles@kln.gov.my or lax.info@kln.gov.my

 Kindly ensure that your email includes the following information:

  • Full Name
  • Contact Number
  • City and State of Residence
  • Subject Line: Deceased Malaysian abroad (please indicate the State)

 

Documents required are as follows:

  1. Original Death Certificate
  2. Original Certificate of Disposition of Human Remains
  3. Original Cremation Certificate (if any)
  4. Name and Address of Mortuary
  5. Original Passport of Deceased
  6. A copy of passport for the person accompanying the body / ashes
  7. A copy of Flight itinerary/ticket.
  8. For return of your passport and documents, provide one (1) prepaid Priority Mail Express Flat Rate Envelope (USD30.45) available from your local U.S. Postal Service.
