Children born in the United States of America can be registered at Embassy of Malaysia, Washington D.C. on these conditions: 

 

- Father of the child is a Malaysian Citizen

- Registration is done within one year from the date of birth

- Registration must be done by appearing in person (with child) at the Embassy of Malaysia, Washington D.C.   

 

Requirements:

 

- Passport of parents 

- Identification card of both parents. (If the mother is not Malaysian, passport will be used as an identity)

- Malaysian and U.S. Marriage Certificate

- Birth certificates of both parents 

- U.S. birth certificate of child

- Two (2) passport size photographs 

- Postage stamp worth $19.99 to mail the certificate to you.

- Fee - **USD8.00  

- To request for the registration form please provide self-addressed envelopes with sufficient stamps to the Embassy

 

The Embassy of Malaysia will not be responsible for passports/documents lost in mail.
