Birth/Marriage Registration
INSTRUCTIONS FOR REGISTRATION OF MARRIAGE
AT EMBASSY OF MALAYSIA, WASHINGTON, D.C.
You must register your marriage at the nearest Malaysian mission in your jurisdiction (please see below). If you are within the jurisdiction of Washington, D.C., you may email in your preferred date to check availability (Monday-Friday; time is usually set at 10.00 am for all appointments) to consularwdc@gmail.com / consularwdc@kln.gov.my.
You are advised to register your marriage within the first six months or it will be considered as late registration and there will be a penalty fee imposed. You and your spouse are required to come in person to register your marriage.
Please make sure you follow the requirements below:
1. Your USA Marriage Certificate, Spouse (Non Malaysian) Birth Certificate and Passport MUST be notarized/certified true by a Notary Public first
2. Then you are required to bring all of these documents during your appointment with the Consular Section:
For Applicant (Malaysian):
- Original and 2 copies of Malaysian Passport (information page only)
- Original and 2 copies of Malaysian National Identity Card
- Original and 2 copies of Malaysian Birth Certificate
- Registration of Malaysians in USA Form
For Spouse (Foreigner):
- Original and 2 copies of spouse original Passport (information page only)
- Original and 2 notarized copies of spouse Birth Certificate
(please provide all relevant supporting documents if name in Birth Certificate and Passport differs)
Other documents required for both:
- Two (2) recent passport size photographs each
- Original and 2 notarized copies of USA Marriage Certificate
- Wedding photos or invitation wedding card (if available)
- - Fees applicable - (fee will be calculated based on your appointment date)
- For return postage - USPS Express Mail only.
(envelope, label and one single stamp of US$26.95 available at your local Post Office)

INSTRUCTIONS FOR REGISTRATION OF BIRTH (BORANG H)
AT EMBASSY OF MALAYSIA, WASHINGTON D.C.
You must register at the nearest Malaysian mission in your jurisdiction (please see below). If you are within the jurisdiction of Washington, D.C., you may email in your preferred date to check availability (Monday-Friday; time is usually set at 10.00 am for all appointments) to consularwdc@gmail.com / consularwdc@kln.gov.my.
Please be advised that both parents and newborn child MUST be present at the Embassy of Malaysia to register the birth of your newborn child within one (1) year. Kindly email us to set up an appointment at your earliest convenience.
To obtain a Birth Certificate (Borang H), please make sure you follow the requirements below;
1. Your Child’s USA Birth Certificate MUST be notarized/certified true by a Notary Public first
2. Then you are required to bring all of these documents during your appointment with the Consular Section;
- Original and two (2) notarized/certified true (original copies) of child’s USA Birth Certificate.
- Original and two (2) copies of parents Malaysian Identity Card.
- Original and two (2) copies of parents Birth Certificate.
- Original and two (2) copies of Malaysian Marriage Certificate.
- Original and two (2) copies of parents Malaysian Passport.
- Fee of USD8.00 by US Postal Money Order made payable to “Embassy of Malaysia”
- Two (2) recently taken passport size photograph of the child (white background)
- For return postage – USPS Express Mail only
(We require envelope, label and one single stamp of US$26.95 available from your local Post Office)
IMPORTANT
Instruction by Jabatan Pendaftaran Negara Malaysia:
Both parents must be present with their newborn child at the Embassy of Malaysia for the Registration of Birth
Note:
1. Registration of new born child should be done within a year from the date of birth. Failure to do so will cause delay in processing and has to be referred to Jabatan Pendaftaran Negara, Malaysia (Registrar of Birth & Death of the Citizens Registrar’s Office) for approval. A letter from the father explaining the reason for the delay in registering is required
2. ‘Borang D’ must be signed only by the father (Malaysian). Mother’s signature is not acceptable; Borang D will be given upon registration at the Consular Section.
