ISSUANCE AND RENEWAL OF MALAYSIAN PASSPORT AT THE EMBASSY OF MALAYSIA, WASHINGTON D.C.

 

At this time, applicant that is within our jurisdictions must submit their passport renewal application via MAIL only. Do note that you also have the option of going personally to the Consulate General of Malaysia in New York to obtain a new passport within one day. Please contact them directly before going (you may need to schedule an appointment); to get instructions and other matters concerning passport renewal should you wish to get your passport done at the Malaysian Consulate in New York.

 

Documents required are as follows:

  • Fully completed, dated and signed IM42 Form (please sign section E. 13(d) and NOT section F. 14)
  • Current original Malaysian Passport
  • Two (2) clear original copies of current Malaysian Passport (page with your details only i.e. photo, name, passport no, etc)
  • Original Malaysian Identification Card (MyKad)
  • Two (2) clear original copies (front and back on the same page) - Old Identification Card (with no chip) will not be accepted. If you do not have a MyKad you will need to get an extraction of your I/C (Cabutan Daftar Kad Pengenalan) from any Jabatan Pendaftaran Negara branch in Malaysia. Any family member can get one for you and the original copy issued by JPN will be needed to process your passport renewal application.
  • Two (2) notarized (clear original copies) of Malaysian Citizenship certificate or card - only for applicants who were born before 1957 and do not have Malaysian Birth Certificate 
  • Two (2) notarized (clear original copies) of US Permanent Residence Card (front and back on the same page) or valid visa status in the USA. For those without a valid visa status, please complete the Statutory Declaration (notarized with a public notary)
  • Three (3) recent passport size photographs - standard 2inch x 2inch   
  1. White background - Make sure picture is of good quality/no shadows/not too dark or light
  2. DO NOT wear any spectacles/colored contact lenses
  3. DO NOT wear white colored shirt/dress - wear something dark
  4. DO NOT wear any hair accessories/large earrings
  5. DO NOT show your teeth
  6. Make sure eyebrows, forehead and ears are clearly visible - push your hair to the back
  7. If you are wearing a headscarf, please makes sure your eyebrows and forehead is seen
  • Completed and signed Registration of Malaysian Citizens in the USA form
  • Contact information form
  • For return postage – USPS Express Mail amounting USD$30.45 (subject to change)
  • 1 envelope, 1 label and 2 stamps amounting to US$30.45 (subject to change) available from your local Post Office) – include all documents in your package to us. Make sure you complete your name and address on the mailing label. DO NOT stick anything on the envelope.

 

For children under 18 years old:

  • Fully completed, dated IM42 Form (either Malaysian parent must sign the application form (IM42) at section 13(d) and below it)
  • Completed and signed Borang A1/B1 (Section A only) - Kindly leave the “Saksi” part blank
  • Current original Malaysian Passport
  • Two (2) clear original copies of current Malaysian Passport (page with your details only i.e. photo, name, passport no, etc)
  • Two (2) notarized (clear original copies) of child’s Malaysian Birth Certificate, Borang W/Borang H
  • Two (2) notarized (clear original copies) of child’s original Birth Certificate (if born abroad)
  • Child’s Original Malaysian MyKid/MyKad and two (2) copies (child 17 years old and above must have) (front and back on the same page)
  • Two (2) notarized (clear original copies) of parents Malaysian Identity Card (front and back on the same page)
  • Two (2) notarized (clear original copies) of parents Malaysian Passport
  • Two (2) notarized (clear original copies) of parents Malaysian Marriage Certificate
  • Two (2) notarized (clear original copies) of parents Malaysian Birth Certificate
  • Two (2) notarized (clear original copies) of parents US Permanent Resident Card/Visa status in the USA (front and back on the same page)

***Passport fees:

All payments must be via USPS Money Order only (available from your local Post Office) made payable to: “Embassy of Malaysia”. Other Money Order or Cashier’s Check will not be accepted.

  1. Children under 12 years - US$35.00 
  2. Students between 19-21 years with F-1 Visa (sponsored by Malaysia Government only – proof of sponsorship required) - US$35.00
  3. Senior Citizen (age 60 and above) - US$35.00
  4. Others – US$70.00 
  5. Persons with disabilities (copy of OKU card required) – GRATIS.

IMPORTANT:

  • Please be sure to place your right or left (either one) thumb print as indicated on the application form IM.42 (use black ink only)
  • Effective April 2023, you will have to submit your original MyKad/MyKid with your passport application;
  • To ensure immediate processing, please make sure that all supporting documents are notarized by a Notary Public;
  • Incomplete application will be returned at your own expense;
  • Submit application at least 6 months prior to expiration date as processing time may take up to 2 months (lost passport/expired passport more than 10 years will take longer to process);
  • The Embassy of Malaysia is not responsible for any delay due to unforeseen circumstances or any loss of application/passport(s) in the mail;
  • Effective 01 May 2012, all passport applications will be forwarded to our Immigration Department at the Consulate General of Malaysia in New York for processing;
