CHILD BIRTH REGISTRATION
Effective 01 January 2008, Child Birth Registration should be submitted in person by either parent (Malaysian Citizen) at the High Commission of Malaysia, Canberra. The office no longer accepts application submitted by mail.
Please make an appointment to submit your application.
Appointment days: Monday to Thursday
Appointment Time: 9:00am to 10:00am
Collection of Document: The same day after 3:00pm
The document for the Application for Child Birth Registration can be obtained after 3pm on the same day.
If you are applying for Child Birth Registration, please lodge the applications before 10 a.m to ensure that both documents are ready the same day.
Please ensure you make enough photocopies of documents required for each application. Refer to passport section for all the required documents.
The father of the child must be a Malaysian Citizen at the birth of the child.
NOTE:
The Father or Mother who is a Malaysian Citizen needs to sign the application form (Borang D) in front of the officer during submission of the application form.
The presence of the child is not required. However the child's thumb print on the form (Borang D) and child's photographs (at the back of the photos) must be witnessed and certified by either the Malaysian Consulate or Honorary Consulate of Malaysia (New South Wales, Queensland and South Australia).
As there is no Consulate or Honorary Consulate of Malaysia in Tasmania, applicants from Tasmania can get the thumb print witnessed and signed by either the Justice of the Peace or a Police Officer .
Child Registration for Birth Certificate before a year old (Malaysian Father)
Required Documents (Bring all original documents and 2 copies of 1-8, no need to be certified. Please sort the documents in 2 separate sets according to the sequence below):-
1. Borang D (refer to download section)
2. Child's Australian Birth Certificate (both sides) issued by Registry of Births, Deaths
and Marriages, Australia
3. Child's Autralian passport (if any)
4. Parents' Malaysian Marriage certificates (It is compulsory to register your Marriage
with the High Commission for Malaysia. For details, please refer to Marriage
Registration)
5. Parents' Birth Certificates (both sides)
6. Parents' Identity cards (both sides)
7. Parents' Passports including visa page
8. Current Certificate of Evidence of Residence Status (6 month validation) or Visa
Entitlement Verification Online (VEVO), if the father is Permanent Resident.
9. 4 recent passport-size photographs of the child (Photographs could be taken at
High Commission's office.)
10. Fee of AUD 25.00 cash or money order payable to High Commission of Malaysia.
Child Registration for Birth Certificate after a year old (Malaysian Father)
The registration takes place after the child turns (1) year old or when the application is received after the child turns a year old.
NOTE:
The Father or Mother who is a Malaysia Citizen needs to sign the application form in front of the officer during submission of the application form.
The application will be submitted to Malaysia. The processing time could take up to 6 months and longer.
The presence of the child is not required. However the child's thumbprint on the form (page 5, Borang E) and child's photographs must be witnessed and certified by either the Malaysian Consulate, Justice of the Peace or a Police Officer.
The father or the mother must lodged the application in person and bring in all the original documents and 2 copies of each document requested below (no need to be certified).
Required Documents (Bring all original documents and 3 copies of 1-8, no need to be certified. Please sort the documents in 3 separate sets according to the sequence below):-
1. Borang E (refer to the downloads section)
2. Child's Australian Birth Certificate (both sides) issued by Registry of Births,
Deaths and Marriages, Australia.
3. Child's Australian passport (if any)
4. Parents' Malaysian Marriage certificates (both sides)
5. Parents' Birth Certificates (both sides)
6. Parents' Identity cards (both sides)
7. Parents' Passports including visa page
8. Current Certificate of Evidence of Residence Status (6 month validation) or Visa
Entitlement Verification Online (VEVO), if the father is Permanent Resident.
9. Four (4) recent passport-size photographs of the child. The High Commission also
provides photograph service from 9:00am to 11am, Monday to Thursday, with a
fee of AUD20.00 for 4 pieces - Cash payment only.
10. Fee of AUD 25.00 made payable to Malaysian High Commission either by Money
Order or Bank Cheque. The office also accepts Cash payment.
Child Registration for Citizenship Certificate before a year old (Foreign Father)
Under item 15(2) of the Malaysia Federal Constitution, starting from 1 June 2010, a Malaysian mother and a foreign father will be able to apply for Malaysian citizenship for their child born overseas. Application can be made at the High Commission / Embassy / Consulate General under the following conditions and requirements:-
(i) application must be submitted within a year from the birth of the child;
(ii) the mother is a Malaysian when the child was born; and
(iii) the child was born in Australia after the date of legal marriage.
The High Commission / Embassy / Consulate General is not allowed to register a child who was born before 1 January 2010. However, if the parents wish to do so, the Malaysian mother of the child is advised to check with Jabatan Pendaftaran Negara Malaysia (JPNM) / www.jpn.gov.my.
Both parents will have to appear in person before the Consular officer at the High Commission / Embassy / Consulate General when submitting their application.
The mother, who is a Malaysian Citizen, needs to sign the application form (Borang B) and Lampiran A.15(2) in front of the officer during submission of the application form.
The presence of the child is also required. The child's thumb print on the application form (Borang B) and child's photographs (at the back of the photos) must be witnessed and certified by the High Commission.
Required Documents:
Bring the all original documents and 3 copies of required documents below, no need to be certified. Please sort the documents in 3 separate sets according to the sequence below. Borang B and other supporting documents need to be printed on both sides (double sided print) and arranged according to the order as listed below:-
1. Borang B ( Borang B.pdf ) and Lampiran A.15(2) (Lampiran A.pdf )- which is a
joint-declaration form.
2. Child's Australian Birth Certificate (both sides) issued by Registry of Births, Deaths
and Marriages, Australia.
3. Child's Australian passport (if any).
4. Parents' Malaysian Marriage Certificates (it is compulsory to register your Marriage
with the High Commission of Malaysia. For details, please refer to Marriage
Registration section).
5. Parents' Birth Certificates (both sides).
6. Parents'/Mother’s Identity Cards (both sides).
7. Parents' Passports including visa page.
8. Father's Citizenship Certificate.
9. 4 recent passport-size photographs of the child (photographs can be taken at High
Commission's office).
10. Fee of AUD 10.00 cash payable to Malaysian High Commission.
For Successful Application, Applicant and child will be required to go to the High Commission/Embassy/Consular General to:-
(a) Pay a fee of AUD $35 cash payable to High Commission of Malaysia; and
(b) Complete Lampiran A-WN ( Lampiran A-WN.pdf in 3 sets).
Please bring along child's Australian birth certificate, 2 recent passport-size photographs of the child and parents’ identification document.
When ‘Sijil Warganegara’ is ready for collection, applicant will be required to go to High Commission / Embassy / Consular General again to collect and complete Lampiran C.15(2) ( Lampiran C.pdf ).
Child Registration for Birth Certificate by Single Parent
All applications will be submitted to Malaysia. The processing time could take up to 6 months and longer. The presence of the child is not required; however the child's thumbprint on the form (page 5, Borang E) and child's photographs must be witnessed and certified by either the Malaysian Consulate, Justice of the Peace or a Police Officer. The father or the mother must lodge the application in person and bring in all the original documents and 2 copies of each document requested below (no need to be certified) .
Required Documents (Bring all original documents and 2 copies of 2-6, no need to be certified. Please sort the documents in 2 separate sets according to the sequence below):-
1. Borang E
2. Child's Australian Birth Certificate (both sides)issued by Registry of Births, Deaths
and Marriages, Australia
3. Child's Australian passport (if any)
4. Mother's/ Father's Birth Certificate (both sides)
5. Mother's/ Father's Identity card (both sides)
6. Mother's/ Father's Passport including visa page
7. Single search document from your state Registry of Births, Deaths and Marriages to
show you are not married in that jurisdiction. This must be attested by your local
State Office of the Department of Foreign Affairs and Trade.
8. Statutory Declaration signed and witnessed and signed by the Justice of the Peace
stating that you are not married in any Australian Territory.
9. Three (3) recent passport-size photographs of the child.
10. Fee of AUD 25.00 made payable to Malaysian High Commission either by Money
Order or Bank Cheque. The office also accepts Cash payment.
Extraction of Birth Certificate
Extraction of birth certificate is not done by the High Commission. You can get an extraction of the birth certificate by sending the application form directly to Malaysia.

Required Documents
1. A certified copy of applicant's identification (passport, birth certificate, etc.).
2. Payment of AUD 5.00 in cash which would cover the fee and return postage.

MARRIAGE CERTIFICATE
Both applicants need to be present at the High Commission.
Documents required (2 copies and arrange according to the order as listed below)
- Form JPN KC.06 (download section)
- Marriage certificate issued by Registry of Births, Deaths and Marriages, Australia
- Couples' birth certificate
- Couples' Malaysian identity card (both sides)
- Couples'passport including visa page
- Two (2) recent passport-size photographs of each person (Photo can be taken at High Commission's office in Canberra.)
Note: In the case where the spouse is non-Malaysian, kindly provide birth certificate and passport.
Marriage that took place less than 6 months fee is AUD 10.00.
Marriage over 6 months - Please call the officer in charge for the fee before lodging the application.
