REGISTRATION OF BIRTH
 
All Malaysians born in Brazil must be registered at the Embassy.
 
Documents to be submitted:-
 
i.    2 copies ofRegistration Form D, JPN.WN02( original)
ii.    3 recent coloured photographs of the child, size 3.5.x 5.0 cm
iii.   Birth certificate of child with English translation or international version (original and 2    photocopies)
iv.   Passport of parents (original and 2 photocopies)
v.    Identity card of parents (original and 2 photocopies)
vi.   Marriage certificate of parents (original and 2 photocopies)
vii   Birth Certificates of parents (original and 2 photocopies)
viii. Fee: Form D, JPN WN02 RM 20.00
      Fee: Form W (W.N.23A-PIN.2/81) RM 10.00
 

Note: The child must be present at the Embassy

 
 
 
 
Registration of Marriage
 
 Application has to be made in person at the Embassy of Malaysia, Brasilia
 
Documents to be submitted:
 
i.           1 copy of Registration Form, JPN. KC 06 can be downloaded at www.jpn.gov.my 
ii.         1 copy of the Form D
iii.     3 recent coloured photographs of applicants, size 3.5 x 5.0 cm
iv.     Brazilian Marriage Certificate with translation in English or international version
        (original and 2      photocopies)
v.      Passports of applicants (original and 2 photocopies)
vi.       Identification cards of applicants (original and 2 photocopies)
vii.       Fee:RM20.00
 
Note:
The marriage must be registered with the Embassy within 6 months from the date of marriage. Any registration made after 6 months of marriage is considered as late registration. The fee for late registration is RM 100.00 *for the first six months and RM 50.00 * for every six months thereof.
 
 
Issuance of Single Status Certificate
 
Applicant has to submit a letter from the National Registration Dept. (JPN) confirming the marital status of the applicant -single or divorced.
 
Fee: RM 20.00 *
 
 
Note:
For Muslim marriages solemnised in Brazil, registration must be done with the Jabatan Agama Islam/Islamic Religion Department in the applicant’s area of residence.
 
The Certificate of conversion and the marriage certificate must be translated into English.
 
The Embassy will then authenticate these documents. (Please refer to the requirements of Authentication of Documents).
