APPLICATION FOR A NEW / RENEWAL OF MALAYSIAN PASSPORT

 

In view of the ongoing developments and guidelines issued by the local authorities related to COVID-19, all immigration services (passport and visa) are provided by appointment only (no walk-in). Please email to newyork@imi.gov.my to make an appointment.

 

 

MONDAY TO THURSDAY: 9.30 AM TO 12.30 NOON (SUBMISSION)

                                         : 2.00 PM TO 4.00 PM (COLLECTION)

FRIDAY                            : 9.30 AM TO 11.30 AM (SUBMISSION)

                                         : 2.45 PM TO 4.00 PM (COLLECTION)

                              

All applicants living in the 17 states under the jurisdiction of the Consulate General of Malaysia in New York as below MUST submit their application IN PERSON:

Arkansas, Connecticut, Delaware, Iowa, Maine, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Oklahoma, Pennsylvania, Rhode Island, Vermont and Wisconsin. (Mail-in applications are subjected to case-by-case basis only. For further inquiries, kindly email to newyork@imi.gov.my.)

 

Please bring/submit the following:

 

1.    Original current Malaysian Passport;

2.    Original and one copy of your Malaysian Identification Card - front and back (or

       certified true copy*);

3.    Original and one copy of Proof of Legal Status in the US;

  (i.e. Permanent Resident Card or Valid U.S. Visa);

4.    For children's passport applications (under 18 years) please include:   

4.1  Original and one copy of the child's Malaysian Registration of birth (Borang W or

       Borang H) or birth certificate;

4.2  Mykid or Mykad of the children

4.3  Original and one copy of either Father's or Mother's Malaysian Identity Card

       (or certified true copy*);

5.   Completed and signed Registration of Malaysian Citizen in the USA form (The

      form can be downloaded from the download section on the website).  This form

      is the requirement from the Consulate General of Malaysia, New York. 

 

*Certified true copies are accepted ONLY from Malaysia if obtained from:

(a) any Immigration office;

(b) any Jabatan Pendaftaran Negara office;

(c) Ministry of Foreign Affairs, Malaysia.

 

Important Note: The photograph for the new passport will be taken as a live-capture photograph at the counter.  Please wear dark colour clothing. 

For women with hijab, please wear a dark colour hijab as well.  Applicants must wear presentable and appropriate attire.

 

Passport fees:

 

6.   With effect from January 15, 2015, please be informed that the new fees for the Malaysian International Passport (PMA) are as follows:


Normal Applicants (Age 13 – 59 years old)              = USD70.00

Senior Citizens -- 60 years old and above                = USD35.00

Children -- 12 years old and below                           = USD35.00

Persons with disabilities with Malaysian OKU Card  = Gratis

 

The payment must be in cash or money order made payable to “The Consulate General of Malaysia

 

* Money order is accepted ONLY FROM:

  i. The United State Postal Service;

 ii. Western Union; or

iii. MoneyGram.

 

 

REPORT OF LOST MALAYSIAN PASSPORT

 

Please bring/submit the following:-

 

1.    Completed Report of Lost Passport application form (Form IM. 203);

2.    Completed Borang Soal Selidik Kehilangan (V) Passport Malaysia (Form IM. 42C);

3.    Completed Borang Permohonan Passport (Form IM.42);

4.    Original and one copy of your Malaysian Identification Card (or certified true copy*) (front and back);

5.    Original and one copy (or certified copy) of your US Permanent Resident Card or any other document verifying your status in the US;

6.    Original letter and one copy from your employer or school to confirm your present employment or studies;

7.    A Police Report indicating the loss of your passport (original and a copy);

8.    One recent passport size photograph;

9.   Completed Registration of Malaysian Citizens in the US form. – Requirement by the Consulate General of Malaysia, New York.

10. Processing times for lost passport will be done in five working days.

 

Note: (V) The Soal Selidik Kehilangan form (IM.42c) is not available for download.  You may collect it personally and you may be subjected to an interview at the Consulate General Malaysia, New York.

 

REPORT OF DAMAGED MALAYSIAN PASSPORT

 

Please bring/submit the following:-

 

1.    Completed Report of Damaged Passport form;

2.    Completed Borang Permohonan Passport (Form IM.42);

3.    Original and one copy of your Malaysian Identification Card (or certified true copy*) (front and back);

4.    Original and one copy (or certified copy*) of your US Permanent Resident Card or any other document verifying your status in the US;

5.    Original letter and one copy from your employer or school to confirm your present employment or studies;

6.    One recent passport size photograph;

7.    Completed Registration of Malaysian Citizens in the US form. – Requirement by the Consulate General of Malaysia, New York;

8. You may be subjected to an interview at the Consulate General of Malaysia, New York.

 

EMERGENCY CERTIFICATE

 

IMPORTANT NOTE:  THE EMERGENCY CERTIFICATE OF MALAYSIA IS ACCEPTED FOR TRANSIT PURPOSES FOR ONWARD TRAVEL TO MALAYSIA IN THESE COUNTRIES ONLY: HONG KONG / TURKEY OR UNITED KINGDOM.  PLEASE ENSURE YOU CHOOSE ONE OF THE ABOVE ROUTES  BEFORE YOU PURCHASE THE FLIGHT TICKET.

 

Applicants who wish to apply for an Emergency Certificate must provide all the above requirements along with the following items:-

 

1.    Copy of your airline ticket from the US to Malaysia;

2.  Copy of passport or copy of Malaysian Identification Card or copy of birth certificate;

3.  Original or copy of your US Permanent Resident Card or any other documents verifying your status in the US; (if applicable)

4.    Two (2) recent passport size photographs;

5.    A fee of US $18.00 in cash or money order * made payable to Consulate General of Malaysia.

 

*Certified true copies accepted ONLY from Malaysia if obtained from:

(a) any Immigration office;

(b) any Jabatan Pendaftaran Negara office; or

(c) Ministry of Foreign Affairs, Malaysia.
