i. Completed the birth registration form (form to obtain at Embassy);
ii. Both parents and child to be present at the Embassy;
iii. Birth registration shall be made within one year from the date of birth;
iv. Original passport and photocopy of both parents;
v. Original Marriage Certificate of parents and photocopy;.
vi. Malaysian Identification Card of Parent(s) and photocopy;
vii. Passport of the spouse (Non-Malaysian);
viii. Authenticated Marriage Certificate (by NSO) and Birth Certificate (by NSO) by the Department of Foreign Affairs, the Philippines together with official receipt of payment (original);
ix. One (1) recent passport size photo of the child with white background (3.5 mm x 5.0 mm);
x. Fee : PHP450.00 - Cash
Note: If the parents are not married and the child was born out of wedlock, the child cannot be registered as a Malaysian citizen. The child may follow the citizenship of the mother (foreigner).

REGISTRATION OF MARRIAGE AT THE EMBASSY OF MALAYSIA
A) MARRIAGE CEREMONY HELD IN THE PHILIPPINES
Under the Marriage and Divorce Act 1976, it is compulsory for non-Muslim Malaysians to register their marriage at the nearest Malaysian Mission abroad or in Malaysia within six (6) months after the solemnization of their marriage under Philippines law. In this case, either both or one of the contracting parties must be a Malaysian. Only MONOGAMOUS marriage are applicable under this Act.
Requirements for registration of marriage:
i. Passport of the couple (original document and photocopies);
ii. Malaysian Identification Card (original and photocopies);
iii. Photocopy of passport stamp arrival page into the Philippines at the time of marriage held in Philippines or in other countries (for the couple);
iv. Photocopy of passport stamp of arrival into the Philippines during the re-registration of marriage at the Embassy of Malaysia (for the couple);
v. Marriage solemnization done in third country (other than the Philippines), the marriage certificate shall be translated into English and authenticated by the relevant Embassy in the Philippines before being re-registered at the Embassy of Malaysia in Manila;
vi. Birth certificate of spouse(non-Malaysian);
vii. If the applicant is a divorcee, he/she is required to produce the Absolute Decree or Divorce Certificate (Original and Photocopy) obtained in Malaysia;
viii. If the spouse has passed away (Widow), he/she is required to produce the Death Certificate of spouse (original and copy);
ix. Valid visa / permit of stay in Philippines for the couple (if both are non-Filipino);
x. Marriage Certificate issued by the Philippine Statistics Authority (PSA), Philippines and authenticated by the Department of Foreign Affairs (DFA), Philippines with receipt attached;
xi. All photocopies must be submitted with the originals;
xii Two (2) recent passport size photos of the couple; and
xiii. Fee of PHP300.00 cash

B) MARRIAGE CEREMONY HELD IN MALAYSIA

The Embassy of Malaysia in Manila has no role in issuing the Certificate of Legal Capacity of Marriage if the marriage ceremony took place in Malaysia. Couples are encouraged to visit the Malaysian Registration Department to get further clarification on the process of getting married in Malaysia.
ISSUANCE OF LEGAL CAPACITY LETTER FOR MARRIAGE IN PHILIPPINES FOR NON-MUSLIM MALAYSIAN COUPLES
Requirements
1. Original and photocopy of Statement of Marital Status from the National Registration Department, Putrajaya, Malaysia endorsed by the Consular Division, Ministry of Foreign Affairs, Malaysia;
2. Original and photocopy of Identity Card;
3. Original and photocopy of passport of the couple and;
4. Fee of Pesos 300.00 cash
5. Processing time : 1-2 days
Note:
1. The couple shall be present at the Embassy.
2. After the marriage took place in the Philippines and the couple is issued with a Marriage Certificate by the National Statistic Office (NSO), they shall then:
i. re-register the marriage at the Embassy of Malaysia within six (6) month from the date of marriage. All documents originated from the Philippines i.e. Marriage Certificate, Birth Certificate shall be endorsed by the Department of Foreign Affairs, the Philippines before being submitted to the Embassy;
OR
ii. re-register at the Registration Department of Malaysia within six(6) month from the date of marriage. All documents originated from the Philippines i.e. Marriage Certificate, Birth Certificate shall be endorsed by the Department of Foreign Affairs, the Philippines and re-endorse at the Embassy before it can be used in Malaysia.

ISSUANCE OF LEGAL CAPACITY LETTER FOR MARRIAGE IN PHILPPINES FOR MUSLIM MALAYSIAN COUPLES
The Embassy has no mandate to register marriages of Muslim couples. However, Embassy will be issuing the Legal Capacity for Contract Marriage to the Muslim couple in order for them to get married in the Philippines. Checklist for the issuance of the letter is as follows:
1. Original and photocopy of approval letter to get married overseas from the relevant Religious State Department in Malaysia;
2. Original and copy of identity of both applicants;
3. Original and copy of fiancee conversion certificate;
4. Original and photocopy of passports and;
5. Fee of Pesos 300.00 (cash)
Note:
1. The couple shall be present at the Embassy;
2. After the marriage in the Philippines and the Marriage Certificate has been issued to the couple by the National Statistic Office (NSO), all documents originated from the Philippines i.e. Marriage Certificate, Birth Certificate shall be authenticated by the Department of Foreign Affairs, the Philippines and endorsed at the Embassy before it can be used in Malaysia. The Embassy will be issuing a certification letter of Muslim Marriages as part of the requirement by the religious body in Malaysia.
