Registration of Birth of a Child whose Father is a Malaysian Citizen
v A child born in Japan whose father is a Malaysian citizen of Malaysia can be registered at the Embassy within 1 year of the child’s birth.

v How to Apply: Registration must be made in person at the Embassy. Please bring your child with you during registration.
Registration hours: 9:00 am ~ 12:00 pm
(The Embassy is closed on Saturdays, Sundays, and public holidays)

v Processing time: 1 – 3 days.
[Application for a Malaysian passport can also be made for the child. For the procedure, please refer to Application for New Malaysian Passport the processing time of which is 2-3 months with a fee imposed]
v Malaysian Passport and 1 photocopy of pages with personal information, passport number, status of residence and signature of holder
v Malaysian Identity Card and 1 photocopy of both sides
v Malaysian Birth Certificate and 1 photocopy
v Certificate of Marriage issued by the Government of Malaysia and 2 photocopies (Islamic Certificate of Marriage issued by the Malaysian Islamic Authority for Muslims)
v Certificate of Alien Registration issued by the City Hall (not required if the applicant does not reside in Japan)
v Power of Attorney letter from father (in case the mother is not Malaysian and the comes to register the child in the absence of the child’s father)
[If the father does not reside in Japan, the original copies of the above documents are not required to be submitted. It is sufficient for photocopies of the documents to be submitted. However, these photocopies must be certified true by Ministry of Foreign Affairs of Malaysia or the Embassy or Consulate of Malaysia where the father resides]
v Passport and 1 photocopy of pages with personal information, passport number, status of residence and signature of holder
v Malaysian Identity Card and 1 photocopy of both sides (if mother is Malaysian)
v Malaysian Birth Certificate and 1 photocopy (if mother is Malaysian)
v Resident Card after the birth of the child is registered (if mother is Japanese)
v Certified copy of Family Register after the birth of the child is registered (if mother is Japanese)
v Certificate of Alien Registration issued by the City Hall
v Certificate of Birth issued by hospital and 1 photocopy or Certificate of the facts mentioned in the Report of Birth issued by the City Hall
v Certificate of acceptance of Report of Birth issued by the City Hall
v 2 photographs measuring 5cm x 3.5cm with a light blue background (photo of the face of the child with eyes opened)
